I speculate that a plumbing issue resulting in a hospital stay and severe pain would be from a perforated bowel. Given her chosen line of work, probably came from a tree branch or something like that. Hopefully the doctors will make sure they wash out all of the tree bark so she can quickly return to degrading herself with more household objects.
