I was under the impression that you had to have this exact 2257 compliant form from all "models" in any adult video.(see attachment i had to change it from a .doc to a .txt not sure if its going to look right) but at the last couple shoots ive done theyve had their own "model release" forms, that were written, by themselves or their lawyers or webmaster, etc. They told me all model release forms are 2257 forms, that they are one in the same. Never take legal advice from a performer. You are correct in so much that you need 2 seperate forms. A 2257 doc and a model release. These should not be combined into one. As for your attached doc, its not bad but it could be better. Its actually better than most that Ive seen non-lawyers draft. BTW - hire a lawyer to look over your contracts/forms.
