I think Ivor has a point. That box cover doesn't sell itself. A lot of people don't like the boob-job look so I'd go for something neutral and not emphasize the silicone.

Likewise her look appears "drugged-out" to me which is not necessarily what everyone wants to see (I'm not saying she is drugged out but rather that's the first thing I think when seeing that sort of picture).

Finally it's not clear from the box what the theme is. In a video rental store a customer is going to spend less than one second on your box in a poorly lit porn store, probably squinting to see something on a floor-level rack, and there will be hundreds of other boxes to scan while looking for something interesting. You don't get much time to make an impression.
