I was curious too.
The fact is that these things are not the normal way things are. That's why they're news. The truth is, in a free society, or even a not so free society, the government can't keep an eye on everyone. Nor can the medical system. People are simply going to slip through whatever kind of screening might be given.
It's going to happen, hopefully not often. You just can't stop everything.
Wait, whoa... back up a sec. The medical system has now let 4 insane motherfuckers go on shooting rampages. Seung-Hui Cho, Jared Lee Loughner, James Holmes, and Nidal Malik Hasan in the past several years. That goes just a tad beyond 'a few falling through the cracks.' That's just the mental health system being completely moribund and useless.
All those motherfuckers had a paper trail and medical histories. I'll bet Jerkules' dirty whore that Holmes has a paper trail too.
