kick the coke out of the d2 receptors with a bunch of seroquel or some typical/atypical antipsychotic, doesn't matter that much and it's sleeping a lot, some anhedonia. the humane thing to do would be to give the dude some low dose of methyphenidate or dextroamphetamine in the short term then assuming he's going to be a bit depressed for a while, put him on say, wellbutrin and cymbalta/effexor because something like prozac is just going to address serotonin and it was dopamine sheen was really fucking with. i think both drugs start acting quickly and should get the three major neurotransmitters moving again.
it's awesome that he's gotten away with this run for as long as he has, but eventually he's going to flip out and think bugs are all over him or do something that's a pure psychotic episode that he'll end up in a locked ward for a few days. then get out and repeat as long as he didn't kill someone.

These days IMO they would put Charlie on Zyprexa or Risperidone. At his level of manic behavior it might be prudent to go with something with anti-psychotic properties. Charlie wouldn't be so high and mighty if he were to be shuffling around under the influence of an antipsychotic med.

Have seen Charlie, have worked with CMI population for more than a decade, and read SM's body of work, and my internet Holmesing (Sherlock, not John Curtis or Oliver Wendell) leads me to quantify that probability as about as close to zero as one can get.
Have worked with a numbers of Charlies sans piles of money, mountains of cocaine, and bevies of porn whores. Lack of impulse control would cause of true manic in this room to engage in highly intrusive conduct, conduct featuring overboard behavior, extreme repitition, lack of boundaries, etcetera.
Look at what Charlie does on camera, bragging about felonious criminal conduct such as possession of substantial amounts of cocaine and and inculpatory admissions of misdemeanor criminal conduct with respect to solicitations of prostitution. This is the type of self-destructive impulsive conduct emblematic of the untreated CMI population.

what happens chemically in the brain of someone in cocaine-amphetamine psychosis is so close to what causes a psychotic episode or weeks of mania. the fact tons of blow happens to be involved, his age, and the lack of psych-ward episodes in the last 20 years doesn't sound like there's any persistent condition that serious.
it's textbook stimulant-related psychosis. take the cocaine away and kick it out of where it's binding to and the run's over if you can stop him from just going and doing more blow.
frankly, you're not going to put him on a mood-stabilizer and something like risperdal unless you want to fatten him up like a goose for pate for no reason. fucking stick him with haldol for a few days if you want to pull the battery out of the guy-he doesn't need things that don't act immediately. seroquel's going to have major advantages over prior gen stuff like risperdal in working in 20 minutes and having the option of a theraputic range so freakishly wide of anything similar(12.5-800mg) that it's going to be flexible AND it's also got major sedative properties compared to well, most shit short of chloryl hydrate and haldol through an iv with fingers crossed he wakes up.
a year from now without any stimulant runs for weeks at a time and no history of going off the reservation without cranking up his dopamine levels like someone whose brain does what a quarter ounce of blow a day does to him were born that way are going to present the same way.
the problem is distinguishing and underlying disorder you'd treat with shit like lithium and risperdal would be the wrong thing when it'll take months before you can diagnose someone who just went on a monthlong coke binge. brain's not going to be close to whatever it's equilibrium is, stimulant removed, to try to diagnose anything that closely resembles the symptoms of what you can do to yourself with the drugs he was doing.
if you were working with people not on a coke/speed binge for weeks and clearly bipolar with sober episodes of mania, you guys are all absolutely right-the thing is you can reproduce the behavior and chemistry out of the blue with heavy stimulant use and an atypical antipsychotic that works fast to act as some kind of full or partial agonist, you can look it up, but you want to shoot the engine block out of the guy with a barrett rather than let him hang around then go suicide bomber before the meds work.

Do you know that he hasn't? 24-7 reportage & social media are a phenomenon of the past 5-6 years - celebrities are (and can put themselves) under a much bigger microscope these days. I know that cocaine and amphetamines can induce psychosis, but Charlie just has a certain facility and flair with language that screams bipolar to me. Tweakers twitch, scratch themselves, and mumble shit about G-men bugging their phones and shit like that, in my experience.
Shouldn't be much longer for Charlie either way, so we'll know soon enough.

Pretty sure, he's a bit hypomaniac by nature but I have issues with patience and ADHD that are indistinguishable. Anyhow people either take their meds or stop when they get up, sheen would have repeatedly spent time at the pavillion with fresh cut flowers and james Taylor to walk around olmsteads landscape. Coke would be consistent with someone able to make a bunch or movies and tv then go loony late in life. If he was that bipolar he couldn't be counted on to work. and prolonged and frequent mcleans stays get picked up, we knew who was hanging out there 30 years ago
