They take insurance money. You don't have to be rich to sign up for this stuff. Just sign over your life insurance policy of $150,000.00 or more and they'll take you.

Using the rule of thumb that you should have approx. 10x your annual salary in life insurance, that means that they take plenty of people below the povery line (or vastly under-insured).
