It's mainly a Scandinavian thing, like Loop said. Last time I saw it around here must have been 15+ years ago. Don't know about some Balkan countries; might be popular there as well. On the other hand, you can still smoke everywhere in that part of Europe.
And I don't think a toothbrush will help much against that kind of discolouring. Gross!
