Burg, she only smiles for pictures. It was rare that you saw a look of happiness on her face in casual living, because she's always miserable or yelling at someone (including her best friend / personal assistant). It could just be all the exhaustion of living her material/fake life in her travels as "Bree Olson".

However, to satisfy your curiousities, not something I can personally/visually corroborate, but somewhere on XPT Smokey mentioned that she was kicking back the Percs pretty heavy. Gotta deal with the pain of being an immoral filthy slut somehow, I guess?
