Being a fan of Gag Factor, I'd prefer to see this put to use in that setting. But this is just some half-assed idea I doubt would ever come to be in any case.

I'd like to see that one used in a good two guy/ one whore senerio. She would just lean her head back while locked in like that while another guy drills her throat. Think that would also work good if she was locked in this position facing front (on her knees, guy setting behind her).

I'm sure there are lots of others that would work well. But I'll just leave it at that.
