Basically, a girl would be at the desk talking to someone on the phone or a speaker phone. A guy would be jacking off in the background behind her, while she's having a conversation. Then the guy would pull here head from the phone and cum on her face and be all surprised. It would end with her telling the person on the phone "I have to call you back later".
