HIV has many variants/subtypes/clades/etc. The combination of drugs used in treatment is targeted at the specific virus the patient has. HIV+ people need to avoid further infection because having two or more variants present may require more drugs that have to be used, and beyond the cost and toxicity is the risk that there might not be a combination of drugs that can be used together or treat every variant present.

If it weren't for the risks of multiple HIV variants the gay porn industry could come out of the closet, admit they're all HIV+ and shoot bareback.

PS. In other words, you can get more HIV positive, and this can be problematic for treatment.
