In the beginning of this season, Bobby said to Tony that you never see it coming when you get wacked, everything is just dark. It seems most likely that the moment Tony sees Meadow walking into Holsten's is when a shootout takes place and Tony likely is killed. Tony's father was killed the same way - in a big public shootout while his son was present. The family was unharmed (as in Godfather 3). The end.
