Looking at the casting process, I can tell you it will probably be an inflatable doll with a plastic 'faceplate' as they are not doing a full head cast (Belladonna would most likely be fitted with a bald cap if they were).

Usually when doing a cast of the face alone, there is some sort of 'collar' to separate the front and back of the head and keep the alginate pooled around the face, but they're not using one here which would increase the chance of Belladonna getting her hair all caught up it in it.
