If companies wanted to be creative, then O-Cards could be really cool...you could have a die-cut O-Card that revealed part of the image underneath, maybe a 'Jekyll and Hyde' type porno could have the naughty and nice versions of the protagonists, and Monstar's suggestions about hard and soft versions makes sense, but I'd say the other way around...soft O-Card that can be stripped to reveal hard sleeve for harder outlets.
Having said that, you could just do a double sided DVD sleeve with hard and soft artwork on opposite sides, and I think it would be cheaper than an O-card. O-Cards could also be used as redeemable coupons, i.e. 'Send in 10 O-cards from our releases this year, and we'll send you a free limited edition collectibe DVD comprised of exclusive scenes you can't get anywhere else'. This would a) develop a mailing list for producers b) add a few sales and c) combat piracy.
But that's if the studios want to get creative... 
Wicked's 'Eye Of The Beholder' features a linticular animation cover which transforms between an all black cover with a slit across it through which you can see Kaylani Lei's eyes, to a revealed cover, where the blackness disappars and you get to see all of Kaylani's face. Something like this could easily be achieved with a black die cut O-Card and a sleeve featuring the revealed image underneath.
But an O-Card that is just an exact reproduction of the sleeve underneath is both lame and needless, as has been correctly pointed out.
I'm surprised so few studios push the boat out on packaging...you can win an AVN award for it, you know!
