Never take legal advice from a performer. You are correct in so much that you need 2 seperate forms. A 2257 doc and a model release. These should not be combined into one. As for your attached doc, its not bad but it could be better. Its actually better than most that Ive seen non-lawyers draft.
BTW - hire a lawyer to look over your contracts/forms. Its worth the peace of mind.
