It could be a certain type of cyst found back there. basically that hole could be a "drain" for the pus to leave the body and you can read all about it here.

It says that they are more common in men and fat people. She certainly isn't fat and I hope to God that she never was a guy. If so, has she worked with Kurt? Oh and if that is a cyst the thought of screwing it makes my stomach turn, but, I'd probably watch it anyways.
