I just read an article that mentioned her. Her real name was Kelly Jean Van Dyke and she was married to Jack Nance, the guy who played Eraserhead as well as appearing in other David Lynch movies. During a storm, he called her from a movie shoot threating to leave her if she didn't clean up her act. She said she would kill herself if he hung up on her. Just then, the storm knocked out the phone lines. Sad story.

You guys are forgetting a child actor who appeared in at least 2 popular movies before becoming a porn star as an adult: Scott Schwartz (aka Master Bates in The Toy). He was also the kid who froze his tongue to a metal pole in A Christmas Story.
