I think he's autistic personally, like that kid on the last episode of St Elsewhere with the Snowball Glass Hospital dreaming the whole 8 year show in his head.

I mean if you can bare to look at Sal for a few minutes, he has a far away depth in his eyes that seems to indicate another plane of thinking...science needs to come up with a device to be implanted inside the brain that records a person's thought process, Sal would be a neverending source of world amusement and entertainment. The whole world would tune in to escape and watch the "Life" inside Sal's brain, like a microcosmic Truman Show, made even more entertaining by having Sal's face in the lower left hand corner of the TV screen to remind viewers of what is actually going on and keep everyone grounded in reality as the "Sal Show" escalates to dizzying heights of entertainment and adventure....Peace in the middle east would be possible cuz everyone would stop shooting each other just to watch this....
