Even so it happens sometimes. A guard locked Dahmer in a room with a huge black dude who was psycho, didn't like whites, and had been abused as a kid (Dahmer was white and had killed some black kids...) Ten minutes later various parts of Dahmer were splattered all over the room...

I don't think anyone was held at fault although it's hard to imagine how that could be an accident.
