There were many rumors that medical evidence suggested Marc had been HIV+ for as long as a year. If that's true then essentially everyone in porn would have been directly or indirectly exposed (not father away than third generation list). If one year is an accurate estimate then essentially every girl who did anal scenes would have been directly exposed, and possibly the majority of all of the girls were directly exposed through vaginal sex.
One key, key difference is that anal, while common, was not ubiquitous at that time and DAP was rare. On the other hand Marc was comparatively small and so did more anal scenes than usual.
EDIT: I removed the estimate for vaginal sex transmission. I should not quote ten-year-old numbers, from memory, that were considered uncertain even then. Probably best to call the CDC hotline and ask – no fancy reading of research papers needed for this.
Unprotected anal sex is much more likely to transmit HIV due to a higher risk of micro-fissures or micro-tearing of tissues not normally stretched quite like that. An old CDC estimate circa 1993 put the added risk of anal vs. vaginal transmission around 50x. I have not found a modern estimate yet. The odds are probably a bit higher yet for really big cocks/little girls, and a little higher for DAP.
Performers didn’t work as frequently then as now. Darren James probably did as many scenes in his last three weeks as Marc did in his last three months.
It’s worth pointing out that no genetic fingerprinting was done to show that Marc & the infected girls were infected from the same source, or that Marc was that source. But I don’t think any HIV+ cases appeared after Marc was unmasked (until Tony Montana much later) so Marc was assumed the source of the continuing infections.
